Judge a company by how it treats its employees, not just by its rhetoric.

A company is as good as it treats its employees. Environmental sustainability is also about employee psychological well-being and company health. Many talk the talk, but few really walk the talk.

The well talked about Toyota efficiency and Kaizen methodologies may make the bookkeeper happy and improve quarterly bonuses for top management, but it also produces the largest turnover rates and highest mental exhaustion rates in the world today. When companies take the human factor out of the equation, they raise the risk of mental exhaustion, kill innovation and quash creativity, not to mention disruption in continuity.

The world economic revolution that has led to globalization and efficiency has forgotten that long-term company sustainability can only be achieved through innovation and good mental health.

Whether you are a little eccentric or possess a certain personality type is totally irrelevant. What does matter, is how you fit into your team, manage your mental health and your well-being within the dynamics of a given team. Preventing Burnout

Burnout or mental exhaustion stems from a complex composite of variables and poor bubble management. It slowly compiles and sneaks up upon you and when it bites, your recovery can last for years, and sometimes, you will never return to the same position.

All the hype about leadership and resilience is worth nothing if you cannot manage your own bubble. Managing the permeability, strength and resilience of your bubble, within any given environment, requires a great deal of understanding and knowledge of thyself, but also the demands within various environments. How do you manage the information you see, what noise disrupts the signals filtered through your lenses, and how are your lenses affecting what you interpret and see?

So why is it so difficult to recover from Burnout? Mental exhaustion is just that. You have surpassed your recharging ability and have succumbed to the onslaught of both external demands and your own capacity to manage your self-expectations. Symptoms are compounded and can include: fatigue, irritability, inability to concentrate, chronic tiredness and loss of will, lack of self-care, staring into nothingness with no mental cognition, anxiety, self-blaming and depressive periods, amongst others. Contributing factors include an apathetic management at work, a poor company culture that expects 110 percent without the ability to recharge, reflect or paus, and even putting the customer before the wellbeing of their employees, because they do not understand that a happy employee will automatically treat their customers well. Burnout is a company risk as it affects your most productive employees and its effects can last for more than 2 years! I often refer to the chronic regressive identity trap (CRIT) where individuals get caught up in a whirlwind of negative thoughts related to their environment, situation or physical wellbeing. In the workplace the implosion of time and space, over-cooked Lean and Agile initiatives all contribute to both psychosomatic burnout and overall company wellbeing. In healthcare, patients with chronic ailments identify with their somatic illnesses reinforcing defenses that focus on protecting the ailment rather than managing a way forward. Some people make an effort to move forward whilst others dwell and ferment their negative or thought routines without actionable resolutions.

If your corporate wellbeing does not include proactive psychological methods then you are simply playing Russian Roulette with your employees. Burnout not only affects your most productive employees, but its effects can last for many years. In the U.K. psychological illnesses cost over £100 billion annually. In Sweden, Skandia Insurance estimated psychological illness costs to be 32 billion Kronor annually. There are only 5.01 million employees in Sweden, meaning that each employed Swede needs to pay 6275kr extra in taxes to pay for these costs. Current trends show an unabated 14% increase in psychological ill-health.

Balancing Lean & Wellbeing

Long-term efficiency cannot be achieved without wellbeing. Real and sustainable efficiency is a complex logarithm requiring both industrial and human measures. Any 101 student can cut the fat out, but the key is to really understand the long-term consequences to innovation, growth and human sustainability in relation to time pressure.

Firstly, I would like to thank you all for the 307 e-mails that I received so far in regard to my last blog post. I was surprised by the number and how passionate you all feel about your working environments, a record for one of my posts. I agree wholeheartedly with you and thank you for your examples and for confirming what we already know to be true. There is a real disconnect between employee wellbeing, Lean initiatives and Corporate Objectives, nonetheless, not all are detrimental to employee health and wellbeing. I also understand why many have elected to write e-mails to me instead of commenting online, that is just fine with me, and feel free to continue to do so in the future. I also understand that even in a modern society, it is not easy to raise discontent. I will certainly keep your e-mails confidential. After all, finding a balance with My-E-Health will benefit all parties and stakeholders.

I must also admit that I am a statistical nerd and also believe in evidence-based measures to make decisions. I also realize that some Lean Initiatives seem to have taken Kaizen budget cutting to an extreme with focus on fanatically improving process without regard to parameters affecting the human psychological wellbeing within their equation. Not only is this evident in our own stats but in the 307 e-mails received.

For organizations to be competitive and improve, initiatives must come from within. Management has the obligation to insure the existence of the right environment. Innovation does not just happen. Management should insure that the environment nurtures empowerment. Empowerment will eventually lead to a creative willingness to reach for the stars and make suggestions. Mechanisms to recognize excellence and to proactively enhance psychological wellbeing are key and instrumental. These should provide your teams with the space to think and grow. Providing time to read and reflect on Standard Operating Procedures (should all be available if ISO certified) and by interacting with other team members on a creative basis will all lead to smooth transitions.      

If employee psychological health and wellbeing are to improve, an equilibrium between Lean Workplace Changes and Human Psychometric Measures needs to be implemented. The pendulum has swung too far to the efficiency scale on the right and needs to center. Psychological ill-health (depression, anxiety and stress) is now responsible for more than 50% of all lost sick days and 90 percent of any reduction in performance.

If quality and sustainability are important to your organization, then change measures should be accompanied by psychometric data. If Lean change or workplace conditions are affecting performance or wellbeing of your staff then any enlightenment to the fact should be extremely beneficial. Change always brings increased stress and defense routines and lower motivation, however, if successful these psychological measures will settle and improve by the next quarter. Knowledge of these fundamental psychometrics provides a timely advantage.

Lean Six Sigma & Agile might synergize when it comes to cutting out the causes of waste or defects of over-production, non-utilized talent, transportation inventory, and extra-processing time, amongst others. Nonetheless, they have failed miserably when it comes to human sustainability and employee psychological wellbeing. The implosion of time and space has led to the time pressure felt by the vast majority today. Is this a ticking time-bomb?

Management after all should be about long-term growth not short-term fixes at the cost of growth or wellbeing. When managers lose sight of what management is really about, they lose the ability to feel the pulse and culture of the company.

Inclusion is management using team dynamics built upon a foundation of wellbeing. Likewise, psychological wellbeing is driven by inclusion supported by cognitional theory and knowledge management. Knowledge management however, is not just about understanding columns in an excel sheet, but rather it is about understanding how the situational demands for growth and human sustainability synchronize with inclusion strategies related to team strengths. Consequentially, team strengths rely on composite competencies of emotional intelligence, tacit and explicit knowledge, and also individual abilities within a given changing environment. Things like communication, teamwork, participation, task accountability, stress, motivation and behavior are variables affecting potential conflict and team dynamics.

Environmental Health & Teamwork?

If gone unabated the psychological battle between external demands and your own self-expectations will affect your health and company performance. Company Culture, Mental Health and Wellness

An organization is its employees together with the embedded corporate culture, everything else, is just a consequence thereof. Management is the science of managing your corporate resources effectively and in a sustainable manner.

Without employee sustainability, company foundations only falter as time and space continue to implode. This implosion only increases the perceived time pressure and stress that erodes employees well-being and weakens team dynamics.

Subsequently natural defense routines form within the company culture leading to a loss in productivity, poor organic growth, stifled innovation and so on. Management therefore has a responsibility to all company stakeholders to maintain the equilibrium between cash-flow and well-being.

The numbers are in and they are alarming. Psychiatrists and Doctors have never, in the history of mankind, prescribed as many anti-depressants, anxiety relievers, anti-psychotics and sleeping tablets as they do today. The modern world as we know it does not run on gasoline, but medications instead. 30% of American and European adults (one sixth of the workforce) consume legal prescription medications daily, 10% use illegal drugs (at least once per month), and other natural defensive routines to time pressure lead to an increase of alcohol consumption. As pressure increases so too will the need for fight or flight solutions. Those looking to fight use stimulants to enhance their Dopamine need whilst those in flight withdraw seeking their Serotonin needs through binge eating, alcohol or anti-depressants. The status quo is not acceptable and at My-E-Health our mission is to fight this fight.   

There are those that would profess that increases in mental health are only related to improved self-awareness and up-to-date statistics and that normality really reigns. I suggest that these people either have their heads buried in the sand, or are regular users, or that they are just plain ignorant.

Can chronic pain and wellness coexist at the same time?

What is chronic pain and can it be effectively managed without opioids and anti-depressives? Can we learn to adapt our minds and bodies to deal with overly excited and exhausted neurons? How can we manage the pathophysiology of chronic pain in our lives and how does this understanding lead to my personal plan? How can I challenge the psychology of time versus pain and what coping skills can be learnt to empower me to adapt to new routines, such as body and mind relaxation, and a form of exercise if possible?

About 55% of the world population lives with some form of chronic pain and the older we get the more prevalent it seems to become. Just getting out of bed and going to the bathroom seems like a chore for many with chronic pain. Chronic pain lasting longer than 3-6 months is framed in many diagnoses from cancer to HIV/AIDS to skin conditions, Arthritis in all its forms, back pain, knees, shoulders, feet, hips, and in the direst of cases terminal. All pain uses the same communication circuitry to our brains and is independently interpreted by thresholds. Coping and treating these varying thresholds is not easy.

Chronic pain treatment is as diverse as it is effective. Doctors use everything from muscle relaxants, anti-inflammatories, to opioids, anti-depressives and many others. Opioids are often standard for terminal and other acute (first 2 weeks) pain treatments, however, there are consequences in long-term use. The opioid addiction crisis is now an epidemic all over the world. It is easy to write a prescription and move on to the next patient, but is that in the best interest of the patient? Too much pain stress without the ability to recharge causes many psychosomatic symptoms that can lead to everything from depression, anxiety, and in extreme cases suicide. Should opioid prescriptions be restricted to a short list of diagnoses? It is certainly a slippery slope and extremely hard to balance. Chronic pain is also linked to exhaustion that can be associated to a form of burnout or chronic stress gone too far.

Introduction
The therapeutic package developed by My-e-Health for the treatment of various mental disorders, from fairly moderate to very difficult, consists of three components: (1) The test battery Empowerment for Participation (EFP), (2) Cognitive Behavior Therapy CBT) online, and (3) Flotation REST.
        One purpose of the first component (EFP) is to detect early symptoms, which facilitates treatment. Another aim is to provide an adequate description of problems, irrespective of severity, in order for appropriate treatment methods to be introduced and for properly monitoring and evaluation. The tests included in the EFP represent a compilation of a range of well-tested test techniques that are also found in other psychological tests. The new with EFP is that these well-tested techniques have been combined into a whole where the different parts are allowed to mirror and deepen each other in order to provide a better overall assessment of an individual's psychological conditions.
        The second component of the treatment package is "CBT online" or after floating. International studies show that online counseling, combined with cognitive behavioral therapy, can be used effectively for a variety of clinical problems. Online counseling has also proven to be an effective way to encourage younger people to get treatment because they are often more comfortable using the internet. The evidence for CBT is well-investigated and need not be further discussed here.
        On the other hand, the third component, Flotation Rest (Restricted Environmental Stimulation Technique), may not be as well-known and therefore it will be discussed in more detail below.

Flotation REST a background
Relaxation exercises are an effective method for reducing physiological and psychological responses to stress. To initiate stress-reduction therapy and relaxation training in order to reduce stress and increase the sense of "mindfulness" usually involves great difficulties. It has previously been shown that people in the greatest need of relaxation training are also those who have the most difficulty in implementing and completing such training [1]. Many different techniques (with some scientific support for their effectiveness) of relaxation are described, such as meditation, Tai-Chi, yoga and qigong [2]. These methods often require long and regular practice before benefits become apparent.
        The positive effects that are described in various relaxation techniques may be expected to arise from the deep relaxation achieved, which simultaneously triggers the so called relaxation response (RR) [3]. This is a significant factor because people who have been affected by illness for a long time have also developed a crisis response and, as is well-known, people in acute crisis are not as a rule susceptible to rehabilitation efforts. Not until the re-orientation phase, can patients benefit from more professionally oriented interventions.

Relaxation response - RR
The RR [3] is identified as the physiological opposite of "fight-flight response" or stress response (SR) [4]. The RR is associated with instantly occurring physiological changes that include reduced sympathetic nervous system activity, reduced metabolism, and lowered heart rate, blood pressure, and respiratory rate [5]. At the psychologic level, individuals typically experience RR techniques as genuine rest, recovery, better sleep quality, less need for alcohol and psychoactive medication, as well as an increased sense of control and efficacy in stressful situations [6].
        There are hundreds of methods for change and personal development and many of them have been around for thousands of years while others have been developed in and after the 1970s. Methods as autogenic training [7] physioacustic procedures [8], and biofeedback [6] are examples of techniques developed to induce relaxation and to trigger the RR. Also pharmacological treatment is aimed at triggering the RR, for example, by calming medication. Pharmacological treatments alone rarely succeed in successfully treating stress-related disorders [9].
        In order to successfully trigger RR, at least two main factors are necessary, namely, reduced sensory stimulation and decreased body movements [10]. A crucial and serious problem often observed is that people most in need of relaxation techniques are usually those who have most difficult to participate in and complete the various forms of relaxation exercises - which is the necessary condition to trigger the RR [1]. High stress load also increases the risk of drug abuse. Drinking alcohol can, by example, provide an immediate relaxation for a person in severe stress [11], a factor which may explain why anxiety and stress can often lead to alcoholism. There is a need for safe and effective RR-inducing techniques for persons suffering from high stress load. This situation strongly suggests that new technologies must be developed and carefully evaluated.

Flotation-REST
Flotation-REST (Restricted Environmental Stimulation Technique) is a method that effectively and safely manages to trigger the relaxation response (RR), even in individuals with severe stress problems. Since the flotation-REST method provides an effortless deep relaxation, this is a feasible and valuable treatment for people at an early stage to facilitate the transmission to the reorientation phase. It has been shown that the effect of a rehabilitation program diminishes if delayed too long. Early relaxation therapy treatment can make it possible to introduce rehabilitation efforts at an earlier stage, thus ensuring a faster recovery.
        During the flotation-REST an individual is immersed in a sound and light isolated tank with salt warm water [12, 13, 14]. The water is heated to skin temperature (about 35º -36º C or 95º - 96.8ºF) and is saturated with magnesium sulphate, which is gentle to the skin. The salinity of the water is saltier than the Dead Sea. The darkness and silence during the treatment helps to reduce the sensory impressions from the external environment. In such a sensory isolation attention and thoughts are more and more directed towards 'mindfulness' or the intuitively and visually "here-and-now 'state', i.e., what in psychology is called primary processes. At the same time the secondary processes are reduced, which means that abstract thoughts and ideas about problems or things that have happened or might happen ("then-and-then" thinking) decreases. It is restful and psychologically beneficial to experience recurring episodes of the "just be" state that the flotation tank successfully induces [15]. Similar primary process states of consciousness, but with known and troublesome side effects, can also be achieved using drugs like alcohol, cannabis, LSD, ayahuasca and many others [16, 17, 18].
        The flotation tank is constructed so that the participant has full control and can stop when he/she so wishes. The lid is easy to open from the inside as well as from the outside. By pressing the buttons inside the tank, the participant can turn on a light in the tank or call for staff. Water salinity is very high which gives a good carrying capacity. The high buoyancy in combination with the very low water level (about 3 dm or 12?) gives maximum security. The water is several times filtered and processed automatically between each use, both mechanically and by ultraviolet light. Each participant floats in completely clean water. At the same time it must be emphasized that the flotation-REST is a "mild" form of REST and there are no reports in the literature of hazards or problems to subjects/patients.

Research about flotation tanks - the pioneers of the first wave
Early experiments with sensory isolation reported negative effects for participants [19], but later experiments with REST for a limited time in the flotation tanks (about 60 minutes) elicited a number of positive effects (see below). The method was initially developed in US in order to train pilots and astronauts in sensory deprivation and in "weightless" condition. Sport psychologists discovered that flotation tanks could successfully be used in order to provide more effective control of the negative effects of stress, to enhance the mental training and to improve recovery after strenuous workouts [20]. Results from the "pioneers" of the flotation tank research showed increased well-being and relaxation, mild euphoria, increased originality, improved sleep, and reduced stress and anxiety, as well as that the technique is a suitable complement to psychotherapy. These early research findings are reviewed in PhD-thesis by Kjellgren [12] and by Bood [13]. A meta-analysis made on the basis of pioneering research in 27 studies with a total of 449 participants [21] showed that the floating-REST had positive effects on several physiological measures (eg, lower levels of cortisol and lower blood pressure) and that the method leads to increased wellbeing and improved performance.

Research about flotation tanks - the second wave
The Floatation Tank Research began as a small scale project in Karlstad in 1998 through a partnership with Olympic Support Center in Örebro under Prof. Norlander. A year later they installed the first flotation tank in Karlstad, and in 2003 the Human Performance Laboratory at Karlstad University was inaugurated, with three flotation tanks. The researchers in Karlstad were clear about the need for floating tank research to be carried out according to established methodological principles. Therefore, controlled studies, power calculations and systematic exploration about the various possible placebo effects have been performed; also several qualitative studies have been carried out. Three dissertations have been published with the following themes:

* Sensory deprivation in flotation tank induces an altered states of consciousness and a ?here-and-now? state, i.e. primary processes [12].
* The technique has the ability to trigger the relaxation response even in patients with severe stress and pain problems [13].
* The technique has the ability to trigger the relaxation response even in patients with severe anxiety (GAD) problems [22].

More than 30 articles have been published from the Human Performance Laboratory. Here are some of the results summarized:

* Athletes experience less perceived exertion during marksmanship after floatation and elite archers performed more consistently after the Flotation-REST condition [23].

* Reduction of MHPG (stress related metabolite, 3-methoxy-4-hydroxyphenylglycol) [24, 25].

* In most cases it is sufficient with a program consisting of two flotation treatments a week for three weeks, then one week without floating, followed by a further three-week period (i.e. a total of 12 flotation treatments in 7 weeks) to activate the relaxation response and thus allow recovery of the body and psyche. In most cases there are no major differences in efficacy between 12 and about 30 flotation sessions [26]. One hypothesis is that it is possible to further shorten the number of sessions. For WAD (Whiplash Associated Disorder) often a longer treatment is needed before the pain begins to subside [27, 28]. The difficulty with these patient groups are often experienced increased pain at the beginning of the treatments, and it is important that staff provide extra support and encouragement until the initial phase has been overcome.

*A study of a 12-float program where pain was measured using the PAI (Pain Area Inventory) revealed that 22% of patients became pain-free, 56% showed significant improvement, 19% experienced no improvement and 3% experienced more pain. In conclusion, 78% of patients were helped by the method [29]. Typical effects of 12-float program is that patients' sleep quality improved (18% - 23%), optimism increased (8% - 10%), stress decreased (25% - 31%), anxiety was reduced (25% - 27%), depression was reduced (24% - 32%) and pain decreased (48% - 57%) [13].

*A follow-up study showed that the beneficial effects persisted after four months [25]. A qualitative case study that combined flotation tank treatment with psychotherapy showed beneficial effects for patients 18 months after [30].

* Very good results with the combination of flotation-REST and psychotherapy. Therapists? impression is that this combined treatment is both faster and more effective [30, 31, 32].

* Men and women perceived the extent and frequency of their pain similarly prior to and following the treatment, and both genders reported improvement due to flotation-REST [33]. Similar findings were obtained with regard to anxiety and sleep quality. There were no significant gender or treatment differences regarding energy. There were, however, gender differences on depression, suggesting that women, compared with men, displayed higher depression scores prior to the flotation-REST treatment. The women improved in terms of depression scores due the treatment and then achieved the same levels as the men. For the first time it was shown that both genders improved their ability to endure pain (i.e. higher scores for the upper threshold) following successful treatment.

* Qualitative studies dedicated to analyse experiences of altered states of consciousness during sensory deprivation in the flotation tank have been performed e. g., [34].

* The method does not seem to depend on placebo effects such as expectation [15], attention [29], or personality [35]. It is difficult but not impossible to investigate placebo effects in this area. In the first study participants who had previously had extensive experience of altered states of consciousness (LSD) and participants without such experiences were included. In addition, there was one ?strict? condition and one fantasy-evoking condition where vivid descriptions of what may happen in the tank were presented. In the second study, half of the persons got normal attention in time to meet the staff at the laboratory while the other half received twice as much time with the staff. Finally, in the third study participants with a "highly sensitive" personality were compared with those without this trait. Taken together, these three studies suggest that placebo effects cannot explain the beneficial results obtained. An additional argument that placebo does not play a major role in this context is that very severe anxiety problems like GAD (which is related to post-traumatic stress disorder, PTSD) can be successfully treated by flotation REST [22, 36, 37, 38].

Beating sadness. Find a space and try sensory awareness. Focus on what you can see, hear, smell and feel. Pay attention carefully to each of your senses in turn. Manage your breathing and try to appreciate the abilities you have been given to experience your surroundings. Get out and find your space, any space, where you can let your senses do what they do best. Allow yourself to enter into that space. Involve yourself in the moment. If you are looking at a tree, for example, think of its roots, trunk, branches, and leaves. Appreciate the varying colors of its leaves, how it moves in the wind or breeze, how the sun or light changes as you move your mindful eyes across its surface. Listen to the leaves and image them dancing in the wind. Feel the presence of nature in the tree and smell the surrounding aromas coming from and around the tree. Control your breathing with slow deep inhalations and exhalations, hold your breathing at times. Allow your senses to take over. This ability is one of your human advantages, please try to enjoy it. PTSD as a traumatic event

PTSD is a traumatic event and it is persistently re-experienced in one (or more) of the following ways: 1. Recurrent and intrusive recollections of the event, including images, thoughts, or perceptions. 2. Recurrent distressing dreams of the event. 3. Acting or feeling as if the traumatic event were recurring (includes a sense of reliving the experience, illusions, hallucinations, and dissociative flashback episodes, including those that occur on awakening or when intoxicated). 4. Intense psychological distress at exposure to internal or external cues that symbolize or resemble an aspect of the traumatic event. 5. Physiologic reactivity on exposure to internal or external cues that symbolize or resemble an aspect of the traumatic event.

Persistent avoidance of stimuli associated with the trauma and numbing of general responsiveness (not present before the trauma), as indicated by three or more of the following: 1. Efforts to avoid thoughts, feeling, or conversations associated with the trauma. 2. Efforts to avoid activities, places, or people that arouse recollections of the trauma. 3. Inability to recall an important aspect of the trauma. 4. Markedly diminished interest or participation in significant activities. 5. Feeling of detachment or estrangement from others 6. Restricted range of affect (e.g., unable to have loving feelings). 7. Sense of foreshortened future (e.g., does not expect to have a career, marriage, children, or a normal life span).

Persistent symptoms of increased arousal (not present before the trauma) as indicated by at least two of the following: 1. Difficulty falling or staying asleep 2. Irritability or outbursts of anger 3. Difficulty concentrating 4. Hypervigilance 5. Exaggerated startle response
